Parramatta
Parramatta is Sydney’s second CBD and sits next to the Westmead health precinct, one of the largest concentrations of hospital and clinical services in Australia. That proximity matters more than people expect. A great deal of NDIS coordination in this area starts with a hospital discharge.
Overview
We coordinate across Parramatta, Harris Park, Westmead, Granville, Merrylands, Wentworthville, Telopea, Dundas and Toongabbie.
What we see
Harris Park and the surrounding suburbs have one of the largest Indian communities in Australia. We coordinate in Hindi, which changes what families are willing to tell us and how quickly a plan starts working.
The other pattern is clinical. Participants leave a Westmead admission with an NDIS plan and a discharge summary, and the two never meet. Reports written by excellent clinicians get set aside by delegates because the report explains the condition without explaining the support.
Our director is an Accredited Social Worker practising in acute inpatient psychiatric care. Translating clinical evidence into something a delegate will act on is the specific thing we do.
Local knowledge
We work across the mental health and rehabilitation services in the Westmead precinct and understand how discharge planning actually runs there.
In Parramatta we provide support coordination, specialist support coordination and psychosocial recovery coaching.
Working with Vitalya
We never coordinate and deliver direct supports for the same participant. No conflicts of interest, ever. We work with plan-managed and self-managed participants. Coordination is available in English, Hindi and Nepali. All NDIS supports are GST-free under section 38-38 of the GST Act.
