We coordinate across Hornsby, Waitara, Asquith, Turramurra, Wahroonga, Pymble, Thornleigh and Berowra. Our practice is based in this part of Sydney, which makes it the area we know best of all.

The upper North Shore is assumed to be well-serviced, and for straightforward supports it mostly is. The assumption breaks at the complex end. Psychosocial disability, autism with high support needs, and plans that involve many providers all need active, clinically literate coordination, and that layer is thin here. Families often tell us they found plenty of providers and nobody holding the whole picture together.

There is also a quieter cohort in this area: adults with psychosocial disability living with ageing parents, where the family has absorbed the support role for decades and the plan has never reflected what will be needed when they no longer can. Planning for that transition before it becomes a crisis is some of the most important work we do here.

Local knowledge

Hornsby Ku-ring-gai Hospital, including its mental health services, is the district’s anchor, and the weeks after a discharge are the ones we prioritise. Being based locally means we can be in the room when it matters.

Coordination is available in English, Hindi and Nepali.

In Hornsby we provide support coordination, specialist support coordination and psychosocial recovery coaching.

We never coordinate and deliver direct supports for the same participant. No conflicts of interest, ever. We work with plan-managed and self-managed participants. Coordination is available in English, Hindi and Nepali. All NDIS supports are GST-free under section 38-38 of the GST Act.
