We coordinate across the Cumberland area, including Merrylands, Granville, Wentworthville, Pendle Hill, Greystanes and South Granville. For Auburn, Lidcombe and Berala, we have a dedicated page for the Auburn area, where we coordinate in Nepali as well as Hindi and English.

What we see in Cumberland

Cumberland sits between two of Sydney’s biggest health precincts, and a large share of the coordination work here starts with a hospital contact. A person is admitted, an NDIS conversation happens on the ward, and they come home to a plan that nobody has activated. The weeks straight after discharge are when supports either start or quietly fail, and that window is the part of the work we treat as urgent.

The area also carries some of the highest housing pressure in Sydney. When a person’s housing is uncertain, every other support becomes harder to hold in place. We coordinate with that reality in front of us rather than around it, and where housing is the barrier, we build the evidence for the supports that address it.

We work alongside the mental health and rehabilitation services in the Westmead precinct and the community services that operate across Merrylands and Granville. We coordinate in English, Hindi and Nepali, which matters in an area where a large share of households use a language other than English at home.

In Cumberland we provide support coordination, specialist support coordination and psychosocial recovery coaching.

We never coordinate and deliver direct supports for the same participant. No conflicts of interest, ever. We work with plan-managed and self-managed participants. Coordination is available in English, Hindi and Nepali. All NDIS supports are GST-free under section 38-38 of the GST Act.
