Support Coordination

Support Coordination is the work of making a plan real. Finding providers who have actual capacity rather than a waitlist. Writing service agreements that hold up. Watching a budget so it lasts the full plan period instead of running out in month seven. Stepping in when a provider stops returning calls.

We are a social work-led practice. Our director is an Accredited Social Worker with a Master of Social Work who currently practises in acute inpatient psychiatric care. That background shapes how we coordinate. We read a plan the way a clinician reads a file, looking for what is missing and what is likely to go wrong next.

We coordinate for adults living with psychosocial disability, autism, intellectual disability, spinal cord injury, cerebral palsy and acquired brain injury. A large share of the people we support have a physical or neurological disability that is complicated by mental health difficulty. That combination is where coordination most often fails, and it is where we are strongest.

We never coordinate and deliver direct supports for the same participant. No conflicts of interest, ever. When we recommend a provider, you can be confident we have nothing to gain from the recommendation.

We work in English, Hindi and Nepali.

Support Coordination is funded from your Capacity Building budget. We charge at the NDIS price limit. All NDIS supports are GST-free under section 38-38 of the GST Act.

Vitalya is an unregistered provider, so we work with plan-managed and self-managed participants. If your plan is NDIA-managed, we can talk you through your options.
